The game was lauded for having artificial intelligence superior to Gran Turismo 4. When compared to Gran Turismo 4, Alfa Romeo Racing Italiano has far fewer cars and tracks. All the cars in the game are manufactured by the Alfa Romeo motor company. While most of the tracks are in Italy, a few are in other countries like Germany and the United States. A distinguishing feature of the game is that it models driver development using a system almost identical to a role-playing video game. In addition, cars get damaged, both visibly and in performance, by collisions or by driving off course.

The game continues the survival horror gameplay of its predecessors: the protagonist explores the surroundings, fighting monsters and solving puzzles. However, it is the first game in the series to do away with pre-rendered backgrounds using a fully polygonal 3D engine instead, which allows for features such as real-time lighting and camera movement. Still, such movements are only used sparingly, and the use of perspective is mostly unchanged from earlier installments: the camera automatically switches to different static angles showing protagonists, monsters and rooms from different spooky viewpoints.

The locals featured in this game include both the familiar (The Raccoon Police Department) and the exotic (A mutated jungle at mid-day) ones. The game boasts a hefty amount of unlockable material such as artworks, notes, trivia, and character costumes. The game is backwards compatible with your save file from Resident Evil: Outbreak, meaning that you can take your unlocked content to this new setting.
