There's also Wizardry 6 and 7, with 7 being open world as well from what I recall.

(Wizardry 1-5 exist, but do not appear to be buyable digitally anywhere (apparently some copyright/trademark issues?), though those games are not open world; each of these games takes place entirely in a single dungeon with a menu-bqased town (except that 4 doesn't have the town). Note that 2, 3, and 4 are not suitable entry points for the series.)

Open world and IIRC totally area scaled (as any self respecting CRPG should be), good character system, good quest system, good item system, some of the best turn based tactical combat of any CRPG, and nice encounter design.
Just about the only thing I didn't like was having to explore towns; they should have been menu towns instead. But that's a minor quibble.

EDIT: Oops, sorry, it's not on GOG. :-(

Have you tried the Spiderweb company's games like: Avernum(both the old ones and the new remakes), the geneforge series, and the avadon series?

Also the Eschalon games might fit the bill and be good for you as well.

I second that recommendation....it is wacky scifi fantasy goodness, though the mobs of enemies can be tricky.

Could quite easily be missing it, but I haven't seen anyone on this thread mention Spiderweb Software games. I've recently completed Geneforge 1 and, despite needing to lower the difficulty half way through, thought it was brilliant. I think there might be a remake coming soon, but I still think the original is worth playing.

I haven't played any games from the Avernum series yet, but I have played the first Avadon game too, and really enjoyed that, although I would say that the Avadon games are aimed more at people who haven't played many RPGs.

I was going to suggest Mount & Blade, but I can see that it was actually the first suggestion. =)

That being said, it's a bit different than your traditional RPG, so don't go into the game thinking that it's going to be similar to a game like Final Fantasy. It really is a fantastic game, with a ton of customization. Highly recommended.
