First, I thought it might be "Silmarils - Dice Kongdom", since the map shares similarity and the story is told with comparable drawings (see attached picture).

But then, I found this campaign map (see attached picture), and it is indeed from "Fantasy General II"
as thraxman has guessed, already. Which one of the game's iteration (Invasion, Onlslaught, Empire Aflame, or Evolution) I do not know, though, for I have not played it/them.
