For me, it's either Fahrenheit (/Indigo Prophecy), Deus Ex, Mass Effect 2 or Fallout: New Vegas, but I should stress that they are on the list for very different reasons: Fahrenheit for having a terrible story and no gameplay, Deus Ex for a terrible story and present but unenjoyable gameplay, Mass Effect 2 for being the greatest disappointment and New Vegas for the enormous pile of bugs that I encountered in the end of the fully patched version.

To me it was Magic & Mayhem:The Art of Magic a very poorly made Master of Magic clone.
And Heroes of Might and Magic 6,possible the worst game so far in the entire series.

+1 to darthspudius, not for his opinion but for the huge brass balls it took to post that in here. XD Seriously man, my hats off to you, that took guts, dissing Deus Ex in this forum. The only thing more courageous would be to start talking shit about Diablo or Baldur's Gate. :D

Wayne's World (SNES)

and for honorable mention:
Krusty's Super Fun House (SNES)

I recognized these games as steaming turd piles even back when I was a kid - when everything seemed magical.
