The game runs fine at first but after a few minutes the game slows to a crawl. I've updated the drivers for my graphics card and set a lower resolution but it still happens. Does anybody know how to solve this?

If I had to blame something hardware, it would be your video card being a little too weak for the recommended specs. Specifically in this case, the video memory might be too little for the settings you have.

I'm thinking that the game loads the initial textures, but then runs into a memory issue once you've walked around some, causing more textures to load.
Try and set the Texture Memory Size to low, and see if it helps.

That should be enough for medium settings, probably not more, because the GTS 250 is still the G92 GPU that just meets minimum specifications.

djlethal's point about texture size is important. Medium settings use small textures and low texture memory size, so this is usually a good starting point for minimum-spec cards.

Another possibility is overheating. Especially if that card has been in service for a couple years, its cooler may be clogged with dust. Well-designed graphics cards protect themselves by slowing down if they get too hot. A can of compressed air takes care of that problem.
I bought a Sapphire Radeon HD 6870. That solved the problem :-)
