Never experienced this on either of my two playthroughs but then, when I was replaying to see the different endings, I came across it twice.

Basically, Geralt will not move after a cutscene/conversation and the only way to get him to do anything is to cast a sign and then he'll suddenly be controllable again.

You can bring up the quick menu, use the camera, but can't draw weapons or move until you do so.

Anyone else have this issue happen to them before?

YES!!! But in my case it was midplay just after an action...he just freezes...sometimes after a minute or two I can get him back others...no...very frustrating!
I've had this happen a couple times, but it always went away within a few seconds of trying different stuff.

It's almost behaving like there is a stuck key on the keyboard or something. Perhaps it could be triggered if a certain event, like a cutscene, happens while you have a key held down?
Geralt froze midstance for me during a couple of fights - i could only move him aorund like he was in an ice skating ring and nothing else and he would only defreeze when an enemy hit him.
really annoying, seeing all it needs is 3 4 hits for Geralt to die on hard, but thankfully it didnt happen too often.

Yes that is what happened to me!
The only time it happened to me the posision and animations of Geralt froze, but he could still attack even if no animation was played out. So if it happens and there's a foe near, don't stop attacking. ;)
Happens to me a lot, only started happening on my 4th playthrough using the Alchemy tree, seems to happen most often when I try to use a bomb.
I think this is related:

from patch 1.1 readme.rtf (found in root install folder), section Known Issues, Gameplay
- A very random input block might occur when using bombs, during dynamic fistfights, and with some interactions. If you encounter this issue, please load a previous game save.
It seems the input bug comes in varying degrees of seriousness and stubbornness.
Particularly when facing a boss later in the game there have been many reports of this bug happening so severely that nothing short of reloading works.
I wonder if there will be a fix for the last "fix" lol.
Its happened now and again for me but i just drew his sword and it fixed it straight away.
