If you read the entries, Geralt is addressed in the third person, it's always 'he' and 'our hero' and stuff like that, so somebody else must have written it.

My guess is Roche, because you spend quite some time with him in the beginning.

(I'm still in chapter 1 so please don't spoil anything!)

Dandelion/Jaskier/Rittersporn wrote the journal. (He's the voice over in the new cutscenes between the acts too.)

If you read the character entries it's obvious it's Dandelion because he's talking about himself in his own entry.
