for some peeps in here with win10 this could be the answer maybe:
"Microsoft already stated that titles like The Witcher 3, Fallout 4, Tomb Raider, Assassin’s Creed and Metal Gear Solid V are effected but also states it's likely a wider range of games involved."

It would flash up on the screen for a second, then there was an icon in the taskbar for witcher 3, like it was minimized, I click it, came up for 1 second, then minimized again. Again, had to force quit through task manager.
Tried rolling back Nvidia drivers, and still no dice.
I deleted 2 files:
user.settings
user.settings.bak
located here:
C:\Users\MY WINDOWS ACCOUNT NAME\Documents\The Witcher 3
And BAM, now the game loads, but I had to re-do all my settings. I put everything back, including Full Screen Mode in the graphics options. Quit the game, then tried to re-launch.
Same problem, all over again. Icon in the taskbar like it's minimized, and clicking it just pops it up for one second, then minimized again, can NOT play it.
Again,
I deleted 2 files:
user.settings
user.settings.bak
located here:
C:\Users\MY WINDOWS ACCOUNT NAME\Documents\The Witcher 3
And the game works again, and I re-did all my settings except one:
I left it set to Borderless INSTEAD of changing it to Full Screen in the graphics options.
Played COMPLETELY Crash/Freeze free for over 2 hours last night, and it still works just fine today.
Maybe that will help,
worth a try.

I have this issue too (experiencing it for the first time now) after reinstalling the game yesterday. I tried deleting the user settings files in C:\Users\User\Documents\The Witcher 3 but it do any good. Also can't find the game overlay box in Galaxy (i have version 1.1.5 and Witcher 3 version 1.8). Is it significan't that the game is installed on another drive (my E drive) than the one where user settings and savegames are?

Sorry for double post, but: how do I change the folder into which my savegames are created? I believe this is the key to fixing the issue for me.

Had the same problem with Witcher 3 not launching. Tried the Game Overlay solution - didn't work. Tried the 'delete user.settings' - didn't work. What DID work in my case was uninstalling Nvidia GeForce Experience and the latest drivers and installing them clean. After a restart everything was back to normal.

My problem is that I cannot get the 1.8.4 version of the game to launch (which is the version I want play since I do not like many of the changes introduced in 1.10). The 1.12 version launches normally, so I don't think it's an issue with the drivers that is the problem - or is it?
Kind of offtopic, but since the god awful alternative look for Ciri DLC is a mandatory install with version 1.12, is there a way to disable it? And if not - then why the hell not?

Sure, there is an entry on W3's start screen.

Okay WTF.
I have played this game for 300 hours with no problem.
I was playing before dinner. I didn't do anything to my PC or the game, not a thing.
Ate dinner, came back to play and now I'm getting this won't start crap. No error message, no indication of what's going wrong, nothing.
I tried the deleting the GOG client solution and that didn't work.
I tried the deleting patch0 solution and that sort of worked. The game launched but had some pretty weird issues like icons missing and weather no behaving normally.
I then verified the game through GOG and after it reinstalled patch0, same fucking problem.
I really don't understand what the hell happened.
Has anyone properly figured this out. Solution would be great but I also want to know why this happened. It just defies reason.
Thanks

I'll also add that I had to actually re-download all of the files from gog.com, my original files were the May 2015 pre-release files, and those refused to reinstall correctly with the latest patches.
It also fixed my Price of Honor quest, for some reason... (Not documented in patch 1.12.x, so I guess the problem was also with the files.)

I seem to have the same issue. I really don't want to re-install given my connection.
I verified and did the C++ redistribution.

I was having this problem. Turns out it was a add-on mod that I had that changed Triss's appearance. When I uninstalled that one mod, the game started loading properly. I would suggest using Nexus Mod Manager to uninstall all mods and see if it starts. Uninstall, not just disable, disabling didn't fix it. Once I did that, I was able to start enabling mods a couple at a time to determine which ones were causing the problem.

Not sure if this is just coincidence but this happened for me when I started installing something else (origin actually) and began getting error messages related to that.
I had to reinstall Visual C++ Redistributable Packages and then it started working again.
